| VALID US PASSPORT |
- You MUST reside within following locations within the consulate's jurisdiction.
- Your passport must be valid, signed and with sufficient blank pages for any visa stamps.
- The passport expiration date must be greater than 6 months from the date of entry.
- You must include a color photograph (2" x 2") - front face and full view with a light background.
- If your passport does not meet these requirements, you must renew your passport, add pages or obtain a new one from us prior to obtaining the visa.

| VACCINATION INFORMATION |
Applicants who have visited any of the following countries within 90 days prior are required to present a valid International Yellow Fever vaccination certificate:
Angola, Benin, Burkina, Cameroon, Colombia, Congo, Ecuador, Faso, French Guyana, Gambia, Ghana, Guinea, Liberia, Nigeria, Sierra Leone, Sudan, Venezuela.

| WHAT WE NEED FROM YOU |
- Valid Passport for 6 months or more.
- Download, print, fully complete, and sign 1 copy of the Brazil application form (Stays over 30 days, 2 application forms and photos are required)
- You must include TWO color photographs (2" x 2") - front face and full view with a light background.
- Your foreign travel plane ticket, OR your printed travel itinerary.
- Business letter from your company and must meet the following: (click here for a sample)
- The letter must be addressed to the Embassy of Brazil
- The letter must explain the nature of business to be performed.
- The letter must be on company letterhead, signed by a manager at your company.
- A Notary Public must notarize the letter.
- Please fill out and submit our ORDER FORM so that we can prepare your documents.

- Our Standard Service Fee - $ 75.00 (3-4 days to process)
- Our RUSH Service Fee - $100.00 (1-2 days to process)
- Return Mail Fee - $ 20.00 (FedEx) OR you may provide us with your own pre-paid airway.
