| Lost Passport |

| STEP 1. Gather the following requirements: |
|
| A. |
Application Form DS-64 (click here to download). Complete it fully. |
|
| B. |
Provide a brief hand-written statement of how you lost your passport in addition to Form DS-64. |
|
| C. |
Application Form DS-11 Application Form Print the form and take it to the passport acceptance facility that you will choose in STEP #2 but DO NOT SIGN until instructed by the passport acceptance facility clerk. PLEASE NOTE: a barcode MUST appear on the top left portion of your application for it to be valid.

Proof of citizenship. One of the following must be submitted: |

- Original birth certificate
- Certified copy of birth certificate
- A previous U.S. passport (expired over 5 years)
- Naturalization certificate (original)

| E. |
Two passport-size photographs (2X2) - Pictures available at Walgreens, Kinkos, etc...
|
|
| F. |
Proof of departure in the next 30 days. One of the following:
|
|
- Copy of airline tickets
- Copy of itinerary or reservation
- Letter on Company's Letterhead (job or social institution)
|
| G. |
Letter of authorization (click here to download).
This is a basic release form that allows Passport Nation Inc. to hand carry your documents to the passport agency.

| H. |
Proof of Identity
Valid driver's license issued over 6 months ago. If driver's license was recently issued please submit copies of the following 3-4 items:
|

- Work picture ID with date of issue
- Social or financial institution ID
- High school diploma
- Tax forms
- Copy of public records with your name
|
| I. |
Government Fees:
|
|
- One check or Money order payable to the Department of State ($135 for adults, and $120 for minors under 16)
- Second check or Money order for $25 payable to the U.S. POSTMASTER or the CLERK OF THE COURTS (which ever agent is chosen).
(TEMPORARY CHECKS ARE NOT ACCEPTED)

| STEP 2. Complete an Order form (click here) to select the service you require. |

Please note that the process time begins the day we receive your documents.
Add $30.00 return shipping fee (FedEx-UPS-DHL) or provide pre-paid air waybill.
Add $35.00 fee for Saturday Overnight

STEP 3. Locate a passport acceptance facility (click here) closest to you and gather all the requirements
listed in Step 1. |

|
The clerk will collect from you all the documents and the government fees, except for the Passport Nation Order Form and fees from (Step 2). Advise the clerk that you are requesting the expedited process through our company, Passport Nation, as stated in the Authorization Letter. After verification, the clerk will ask you to sign the application and place everything in an envelope and seal it. This sealed envelope will be handed back to you for shipping and must remain sealed.

| STEP 4. Ship sealed envelope along with the Passport Nation order form and fees. |
|
You will place the sealed envelope (containing all your documents,) the Passport Nation Order Form, and Passport Nation fees (if paying by money order) inside the shipping envelope (FedEx, UPS or DHL.)
For credit card payment please complete credit card information on the order form. We accept Amex, Visa, MasterCard and Money Orders.
