| STEP 1. Gather the following requirements: |

| A. |
Application Form DS-82 (click here to fill out form online). YOUR SIGNATURE AND NAME MUST BE THE SAME AS ON YOUR EXPIRED PASSPORT. PLEASE NOTE: a barcode MUST appear on the top left portion of your application for it to be valid.

Expired passport must be submitted |
|
- Passport expired no more than five years ago
- Expired passport was issued no more than 15 years ago
- You were at least 16 years old when your last passport was issued
- Expired passport is in good condition
- The name on passport has not changed

| C. |
Two passport-size photographs (2x2) - Pictures available at Walgreens, Kinkos, etc...
|

| D. |
Proof of departure in the next 30 days. One of these:
|
|
- Copy of airline tickets
- Copy of itinerary or reservation
- Letter on Company's Letterhead (job or social institution)
|
| E. |
Letter of authorization (click here to download).
This is a basic release form that allows Passport Nation Inc. to hand carry your documents to the passport agency.

Government Fees:

- Check or money order payable to the Department of State for $135.00
(TEMPORARY CHECKS ARE NOT ACCEPTED)

Please note that the process time begins the day we receive your documents.
Add $30.00 return shipping fee (FedEx-UPS-DHL) or provide pre-paid air waybill.
Add $45.00 fee for Saturday Overnight

| STEP 3. Ship your documents along with the Passport Nation order form and fees. |
|
You will place the signed application (DS-82), your expired passport, your 2 passport photos, the proof of departure, the letter of authorization, the government fees of $127.00, the Passport Nation Order Form, and Passport Nation fees (if paying by money order) inside the shipping envelope (FedEx, UPS or DHL.)
For credit card payment please complete credit card information on the order form. We accept Amex, Visa, MasterCard and Money Orders.
